The skills needed to help teach people how to manage the day-to-day care of chronic disease will be the focus of a class offered this fall through Macomb Community College’s Center for Continuing Education.
The class, “P.A.T.H. (Personal Action Toward Heath) Leader Training,” will be led by two master trainers from the Arthritis Foundation Michigan Chapter and the Area Agency on Aging 1-B. In the class, teams of facilitators will be trained to deliver PATH to health care consumers in community settings.
PATH is a group health education program designed to help people build the skills they need for day-to-day management of chronic diseases, including but not limited to lung disease, heart disease, diabetes, and arthritis. Designed by Stanford University as the Chronic Disease Self-Management Program, the methods of instruction are designed to facilitate group interaction and participation, encouraging participants to share personal experiences and find solutions to common problems.
The class will be offered 9 a.m. – 4 p.m. Fridays, Oct. 17 – Nov. 17, at Macomb’s South Campus in Warren. The class fee is $195.
