STUDENT INSURANCE

 

As a service to our students, Macomb Community College offers an opportunity to enroll in a Student Health Insurance Plan through Student Assurance Services. This plan is available to any student registered at Macomb Community College and attending a minimum of one (1) on-campus class. Columbian Life Insurance Company underwrites the plan. We are pleased to offer and recommend this plan for all students.

Eligible students and dependents may enroll in the plan prior to the enrollment period deadline date for each term of coverage listed in the brochure. Annual and 1st Term deadline date 10-1-08; 2nd Term deadline date 1-30-09; Summer deadline date 6-1-09. Students can also enroll within 30 days of a qualifying event; i.e., new student, loss of other coverage, marriage, birth or adoption. When a student qualifies for insurance outside of the enrollment period, just call the SAS office for a pro-rated amount to come on the plan. Students can no longer sign up for the plan at any time.

This plan provides substantial accident and sickness coverage at a low cost to the students - $575 annually for a student under age 30; $870 annually for those age 30 and over. Additional major medical benefits are available for $200 annually. Enrollment in this plan provides coverage 24 hours a day worldwide, during the term of the student’s policy. Further details concerning eligibility, costs the enrollment and claim forms are located on the Student Assurance Services website: www.sas-mn.com. Printed enrollment forms are limited but available for those who do not have internet access. Call the Office of Human Resources at 586.445.7897.

An identification card will be issued and sent to you within five working days of the receipt of your enrollment form and payment. This will be your only evidence of official enrollment in the plan. Please carry it with you at all times. Macomb Community College will retain the master policy.

It is important that you consider this program or one of your own choice. Please be aware that the College’s liability insurance does not cover injury or sickness sustained by a student; even if these activities result from class or group participation activities such as intramural sport or drama. We encourage you to review your medical insurance coverage to assure that it is adequate. If you do not have coverage, we strongly recommend that you purchase a policy.
