The forms below are used for processing. Not all students are required to submit these documents. These forms are only to be submitted if requested from the Financial Aid Office. If the student submits one or more of these forms and the form(s) was not requested, it could cause a significant delay in the processing of the student’s application and/or jeopardize the student’s eligibility for financial aid.

Dependency Override Form
If you have unusual circumstances and cannot provide parent income information on your FAFSA due to an abusive situation or other special circumstances, you may be eligible for a dependency override. The form below can be used for this purpose. Appropriate documentation is required to support the situation. Submission of a Dependency Override Form does not guarantee it will be approved.

Appeal Forms
The forms below are for special appeal circumstances. If the student was denied financial aid due to failure to meet satisfactory academic progress guidelines or because the independent student or parent(s) of the dependent student lost his/her job, an appeal can be filed. The forms cannot be submitted until the student's application has been processed and the student is denied financial aid. If the student submits an appeal before processing is completed, the appeal will be returned to the student.
